Former 21 Air staff say they raised safety concerns years before the Miami overrun. The departing safety director emailed the owner in 2021 that "everywhere I looked there are issues." The former head of the pilots' union says he was fired in 2022 for pressing it, and has a federal whistleblower suit. The company disputes the claims.
